Job DescriptionI. General Summary The Pharmacy Liaison Representative is responsible for correctly identifying, labeling, packages and/or preparing the correct medication and dosage for the correct patient according to discharge instruction sheets. Reviews patient charts to insure proper prescription usage by the patient or to determine whether a new prescription and/or refill is needed etc. Makes referral appointments and contacts patients for appointments. Calls retail pharmacies for new prescriptions and/or to authorize refills. Assists with administrative work as needed. II.
